NHR supports a wide range of individuals within the East Midlands with diverse needs specifically supporting individuals who have issues with addiction wanting to maintain abstinence and work towards living independently.

This role would include working with individuals who will be affected by substance misuse within support accommodation environment.
The Role
​​
We are looking for an experienced full-time and Part-time Support Worker to join our team, someone who shares our values and can support adults within a supported accommodation environment. NHR supports individuals who have addiction issues and/or multiple complex needs. We are looking for a support worker within a busy fast paced environment. We pride ourselves on putting the individual first and aiding them to work towards living independently. We offer excellent training within a supportive team environment.

The role will be focused on supporting residents within New Hope Rehabilitation properties to engage with activities and support that will lead to independent living through engagement with the Support Plan objectives. For further details please refer to the Job Description.
To support service users placed within New Hope Rehabilitation properties to engage with activities and support that will lead to independent living through engagement with the Support Plan objectives.
• To act as a key contact for residents family, friends and partner organisations
• Assess and monitor the needs of the resident through support plans (reviewed every month)
• Develop Support Plans with residents using the Outcomes Star as the basis of progression
towards independent living
• Key work sessions to be undertaken with resident to assure progress is monitored and
documented appropriately through relevant documentation
• To ensure residents engage with keeping the property to a high standard engaging in activities
that support independent living e.g. cooking, cleaning, budgeting, managing finances, life skills
sessions
